CareVital In-Home Monitoring System

At Spoon River Home Health Services, we provide patients with an opportunity to improve communication with their medical professionals, by enabling them to send vital sign readings without ever having to leave their home.

The CareVital unit utilizes a phone line and sends vital signs data and subjective symptom information from the home directly to a secure website that can be accessed by healthcare professionals through any standard internet browser.  There is no need for expensive priority software to access the patient information anywhere in the world.  All the patient has to do is take their vital signs and answer pre-selected questions about symptoms over the telephone.

What are the benefits?

The following vital signs can be administered and reported from the comfort of your home:

  • Heart rate

  • Blood Pressure

  • Weight

  • Blood Glucose Levels

  • Peak Flow

  • Oxygen Levels

  • FEV

This in-home monitoring system improves communication between the patient and the physician resulting in:

  • Up to 83% fewer hospital readmissions during critical period

  • Breaks the cycle of emergency care hospital readmissions for patients with chronic illness such as CHF, diabetes, etc.

  • More effective and immediate intervention

Who should be considered for CareVital?

Patients who experience:

  • Recent hospitalization or ER visit for existing chronic conditions.

  • Any condition requiring frequent monitoring or trending of vital signs to facilitate clinical management.

  • Chronic care patients who can avoid admission to alternative living facilities.

  • History of poor compliance with diet, medications, or self-monitoring

What is the cost to the patient?

Medicare-eligible patients that are in a current home care episode and require home monitoring, will receive CareVital at no cost for as long as home care is required.

Patients not covered by Medicare or Insurance have the option of being monitored for a nominal monthly fee.
